What is a sports physical?
A sports physical, or pre-participation physical evaluation, checks whether it is safe to start a sport or training program. It focuses on heart health, past injuries, and conditions such as asthma that may need a plan during activity. What the exam includes
Health history
Past injuries, illnesses, medications, and any fainting, chest pain or shortness of breath with exercise.
Family heart history
Questions about sudden death or heart conditions in relatives, an important screening step.
Physical exam
Vital signs, heart and lungs, vision, and joints and muscles.
Clearance and forms
Completion of the required school or league form, or guidance if further evaluation is needed.
Before the visit
How to prepare
- Bring the school or league physical form, with the history section filled out at home.
- Bring glasses or contacts and a list of medications, including inhalers.
- For minors, a parent or guardian should attend or sign as required.
- Wear comfortable clothing.
A sports physical does not replace a regular checkup. It is a good time to schedule an annual physical too.
Stop and get checked
Anyone who faints, has chest pain, severe shortness of breath or a racing heartbeat during exercise should stop activity and be evaluated before returning. Call 911 if someone collapses during sports.

Can I walk in for a sports physical?
Walk-ins are welcome Monday through Friday, 8:30 AM to 5:00 PM. Calling ahead helps, especially before the start of a season.
What form do I need?
Bring the form required by your school or league. Many Texas schools use a specific pre-participation form.
Is a sports physical the same as an annual physical?
No. A sports physical focuses on safe participation; an annual physical covers broader preventive care.
