Chronic disease management
Chronic Disease Management in Richmond, TX
Steady, coordinated care for diabetes, high blood pressure, high cholesterol, asthma and other long-term conditions, led by an internal medicine physician.

What is chronic disease management?
Chronic disease management is ongoing, planned care for health conditions that last a year or longer and need regular attention, such as type 2 diabetes, high blood pressure or asthma. Instead of treating each flare-up on its own, your care team tracks the condition over time, adjusts treatment, watches for complications and helps you stay as healthy as possible.
At Lifecare Primary Medical in Richmond, this care is led by Samira Khan, MD, an internal medicine physician, together with our physician assistants and nurse practitioner.

Why continuity matters
Why steady, ongoing care makes a difference
Chronic conditions change slowly, and small shifts are easy to miss without a consistent record. Seeing the same practice over time means your care team can compare this year’s labs with last year’s, notice a gradual rise in blood pressure, and recognize when a medication has stopped working as well as it once did.
Many adults also have more than one condition. Diabetes, high blood pressure and high cholesterol often appear together, and each one raises the risk of heart and kidney disease. Managing them together in one place helps avoid conflicting advice, duplicate tests and medications that interact poorly.

Your physician
Samira Khan, MD
Board-Certified Internal Medicine Physician
Chronic disease management, including hypertension and diabetes care, is one of Dr. Khan’s main areas of focus. She is affiliated with Memorial Hermann Katy Hospital.
Her approach centers on early detection, clear explanations and treatment plans that fit each patient’s life.
How it works
What ongoing care includes
Medication review
At each visit we go through your medications and supplements, check doses and possible interactions, and ask about side effects. If a medicine is not working or is hard to afford, we talk about alternatives.
Lab work and monitoring
Depending on your conditions, that can include A1C and blood sugar testing, cholesterol panels, kidney function tests and thyroid tests. We explain what each result means and how it affects your plan.
Home tracking
Home blood pressure readings or glucose logs give a much fuller picture than a single office reading. We will show you what to record and how often.
Lifestyle support
Nutrition, physical activity, sleep, weight and quitting tobacco all affect chronic conditions. We focus on practical, achievable changes rather than a long list of rules.
Specialist coordination
When you need a cardiologist, endocrinologist, kidney specialist or another expert, we arrange the referral, share your records and stay involved.
Follow-up planning
You leave each visit knowing when to come back and what will be checked. Stable conditions may need fewer visits; new diagnoses or medication changes usually need closer follow-up.
Warning signs that need urgent attention
Call our office the same day if your home blood pressure or blood sugar readings are much higher than usual, or if a new medication causes worrying side effects. Call 911 or go to the nearest emergency department for chest pain, severe shortness of breath, signs of stroke, fainting, confusion, or very high blood sugar with vomiting or drowsiness.

Common questions
Chronic care questions
How often will I need to come in?
It depends on your conditions and how stable they are. Many patients with well-controlled conditions are seen every three to six months, while new diagnoses or medication changes usually need closer follow-up.
Can one doctor manage diabetes, high blood pressure and high cholesterol together?
Yes. These conditions often occur together, and managing them as a group is a core part of internal medicine.
Should I bring my home readings?
Yes. Bring your blood pressure log, glucose meter or app, and your medication bottles. They make each visit more useful.
Will I still see my specialists?
If you already have specialists, we work with them. If you need a new one, we arrange the referral and keep coordinating your overall care.
Can I be seen the same day if something changes?
Yes. We accept walk-ins and same-day visits on weekdays from 8:30 AM to 5:00 PM.
